FAILED Ejection Seat - Pilot Killed - Counterfeit Components?

On June 30th 2020, Lt David ?MEZr? Schmitz was killed in an F-16 mishap at Shaw Air Force Base, South Carolina. He was assigned to the 77th Fighter Squadron which is apart of the 20th Fighter Wing. A recent article written by Rachel Cohen and published on Air Force Times highlights information never released to the public until now. ?MEZr?s? ejection seat failed to work properly during an ejection sequence ultimately resulting in his death. A key component of the ACES-II ejection seat is the Digital Recovery Sequencer (DRS) which was determined to be at fault for the ejection seat failure. In this episode we talk about the revelations about suspected counterfeit chips and other components that were discovered during the investigation.

TAP #37: Capt Haden "Gator" Fullam - A-10 Pilot/Demo Pilot

"Gator" is the current A-10 Demonstration pilot. He comes from a family of fighter pilots and his family is full of fighter pilots to this day. 

His biological grandfather, Wayne, was an F-105 pilot and was shot down over Vietnam. The day he was shot down he was flying with the callsign Pistol 01. 

The man he grew up knowing as his grandfather, Roger, was career fighter pilot and was shot down over Vietnam in an F-105. The day he was flying he was flying with the callsign Gator 01. 

Wayne and Roger were squadron mates.  It wasn't until years after the war had ended that Wayne was officially declared KIA. 

Today Gator and his brother Jay "Pistol" Fullam are Hawg drivers as well as his sister-in-law.

#23: F-16 Mishap - Fatality - 30 Jun 2020 Shaw AFB, SC Lt. David "MEZr" Schmitz

On 30 June 2020, Lt David ?MEZr? Schmitz was killed in an F-16 mishap at Shaw AFB.  He was conducting a night mission qualification sortie.  Upon his initial landing attempt his aircraft struck the instrument landing system antennas resulting in significant damage to his aircraft.  Following approximately 30 mins of troubleshooting ?MEZr? attempted to recovery the aircraft.  Shortly after touching down the aircraft began to roll and he initiated the ejection sequence.  Due to numerous failures he ejection seat failed to deploy the parachute contained within the seat

"2Watt" is an A-10 pilot and on July 20th, 2017 he was conducted a low angle strafe during an operational check flight.  The gun malfunctioned which resulted in the canopy being jettisoned and damaging the aircraft's hydraulic system.  

"2Watt" successfully recovered the stricken aircraft to a nearby field.  For his superior airmanship he was awarded the Distinguished Flying Cross.

Less than 30 days after his first Distinguished Flying Cross, "Shiner" was tasked to rescue two army pilots downed in enemy controlled territory.  During the initial insertion, his aircraft received enemy fire, striking his flight engineer.   Over the next five hours "Shiner" and his crew would work to recovery the pilots and conduct a total of eight approaches. 

For his efforts "Shiner" was awarded two Distinguished Flying Crosses.

#017 (Part 1) HH-60 Rescue Pilot: Maj Lou "Shiner" Nolting

In this episode Maj Lou "Shiner" Nolting and I talk about two sorties he flew while supporting operations in Afghanistan in 2011. "Shiner" is an HH-60 Air Force Pavehawk pilot, specializing in Combat Search and Rescue.  On March 29th, 2011 "Shiner" and his crew were alerted to respond to an urgent evacuation of four critically wounded Soldiers.  While setting up for a hoist extraction his aircraft came under heavy effective enemy fire.  Rounds struck the main rotor, fuel tank, and number two engine.  With a loss of power "Shiner" executed a pedal turn and plummeted forty feet in an effort to gain airspeed and save the aircraft and seven crew members.  

Less than 30 days later, "Shiner" was tasked to rescue two army pilots downed in enemy controlled territory.  During the initial insertion, his aircraft received enemy fire, striking his flight engineer.   Over the next five hours "Shiner" and his crew would work to recovery the pilots and conduct a total of eight approaches.

#016 (Part 2) Into the Fire, Saving 260+ People Surround by Fire ? Chief Warrant Officer Joe Rosamond

On the night of September 5th, 2020 two California National Guard helicopter crews rescued hundreds of individuals, many with severe burns and injuries. They were trapped with no means of escape during the Creek Fire.  They navigated mountainous terrain, dense smoke, and darkness for hours. Truly impressive story. For their actions they were awarded the Distinguished Flying Cross by President Donald Trump. Their citation reads:

 

he President of the United States hereby awards the Distinguished Flying Cross to Chief Warrant Officer 5 Joseph Rosamond, Chief Warrant Officer 5 Kipp Goding, Chief Warrant Officer 2 Brady Hlebain, Chief Warrant Officer 2 Irvin Hernandez, Warrant Officer 1 Ge Xiong, Sergeant George Esquivel Jr., and Sergeant Cameron Powell of the California Army National Guard for distinguished acts of heroism and extraordinary achievement in aerial flight on September 5th, 2020.

The brave actions and superior airmanship of these soldiers resulted in the successful rescue of 242 adults and children from the rapidly developing Creek Fire in Central California. Their actions are in keeping with the highest military traditions of selfless service, honor, and personal ? personal courage, and reflects a great credit upon themselves, the 40th Combat Aviation Brigade, the California Army National Guard, and the United States Army.
