In this episode, Max discusses the crash of N1125A, a 1991 ASTRA/GULFSTREAM 1125 SP business jet at Ingalls Field Airport in Hot Springs, Virgina. It was the third fatal business jet crash in the U.S. in just five weeks. Five people on board were killed.

317 Naples Challenger Jet Update & 1994 Challenger Dual Flameout + GA News

In this episode, Max discusses two Challenger jet crashes involving dual engine failures: one in Naples, FL, in February 2024 and another in Nebraska in 1994. The incidents share some similarities.

In the Naples crash, both engines experienced oil pressure warnings within seconds of each other before failing simultaneously. The preliminary report indicates the left landing gear touched down first, followed by the right, before the plane skidded into a concrete barrier. The cabin attendant facilitated passengers' safe evacuation through the tail compartment. The pilots, highly experienced, had a combined flight time of over 35,000 hours. The aircraft, with GE CF34 Series turbofan engines, underwent recent airworthiness inspections and was fuel-soaked post-crash, ruling out fuel exhaustion.

Further examination revealed thermal damage to engine components but no clear cause for the simultaneous engine failures. Discussion with an Challenger jet pilot who listens to the show recalled a similar incident involving a Challenger aircraft that also had a dual engine flameout in 1994.

That crash occurred during a positioning flight after passenger drop-off, after experiencing a dual engine flameout between FL370 and FL410. The aircraft sustained substantial damage during a forced landing at night in an alfalfa field.

Analysis revealed water contamination in the fuel, leading to the engines' failure. Both engines were sent for examination, showing consistent findings with fuel samples. The NTSB attributed the crash to inadequate planning, decision-making, and preflight inspections following fuel contamination.

Comparing the two crashes, both had simultaneous dual engine failures occurring around two hours into flight, and both had a yellow liquid in the fuel. At this point, the cause of the Naples crash is still unknown, though Cockpit Voice Recorder and Flight Data Recorder analysis should provide new clues.

The episode underscores the importance of fuel contamination awareness, and the necessity for thorough preflight inspections and proper response to warning signs.

During the trip, they make stops at various airports, encountering different experiences such as efficient refueling processes at Mitchell, South Dakota, and challenges in verifying fuel availability at other airports. They also navigate through unforeseen weather changes, ensuring safety and efficiency throughout the trip.

The return trip involves rerouting due to weather forecasts, aiming to avoid strong headwinds. Max reflects on minor errors made during the journey, emphasizing the importance of staying alert and making quick adjustments when needed. Overall, the episode highlights the intricacies and considerations involved in planning and executing a long-distance flight in a Vision Jet, emphasizing safety, efficiency, and adaptability throughout the journey. 314 Falcon Jet, N283SA Black Hole Crash in Georgia ? with Rob Mark + GA News

Host Max Trescott discusses the fatal crash of a Falcon jet in Georgia with aviation expert Rob Mark. The NTSB final report revealed a series of factors contributing to the accident. These included misreading a NOTAM regarding the ILS glideslope, difficulty entering the initial approach fix into the navigation system, high and fast arrival at the final approach fix, unauthorized use of airbrakes. It?s also possible that they didn?t realize the ILS approach required flying a procedure turn. They were also flying a black hole approach in dark night conditions with minimal ground lights, but apparently weren?t referencing the PAPI visual indicator.

The captain, aged 73, had extensive flight experience but had undergone retraining due to unsatisfactory performance in certain areas. The first officer, aged 63, had a significant number of flight hours, but received only a Second in Command (SIC) type rating, because of performance issues.

The podcast delves into the transcript of communications between the flight crew and Atlanta Center, highlighting confusion regarding NOTAMs and the approach procedure. The crew, flying a cargo route from El Paso to Thomson, Georgia, requested information on the ILS approach, but there was a misunderstanding regarding the status of the glideslope and localizer. The approach required a procedure turn, which the crew seemingly missed, leading to an unstable approach.

There were delays in programming the initial approach fix (IAF) into the navigation system, possibly due to confusion over the fix's identification. The crew ultimately crossed the IAF at an altitude significantly higher than prescribed, leading to a steep descent to intercept the glideslope.

Max created a software simulation of the final minutes of the flight that revealed a rapid descent rate and an unstable approach. Despite warnings from the captain about being high, attempts to correct the descent were ineffective, ultimately resulting in impact with trees just short of the runway. This was a classic black hole approach, in which there are few lights on the ground before the runway. A Boeing simulator study of a black hole approach showed that pilots consistently crash short of the runway, and so they must have either an electronic or visual glide slope. During this accident, the glide slope was out of service.

312 How ATC Heroes Guided a Cincinnati Piper Pilot to Safety + GA News

On the afternoon of Wednesday, 7 Dec, 2022. The Cincinnati, Ohio area had been IMC all day and a single engine aircraft, based at Hogan Field (KHAO) near Cincinnati was flying instrument approaches in IMC conditions with the help of Cincinnati Approach.   

The aircraft departed Hogan field, located under the northern portion of the Cincinnati Class Bravo, and its first two instrument approaches went relatively well. It flew to the southeast at 4000 feet, and was given vectors to the RNAV (GPS) 3 Right approach @ KLUK, which is Lunken Field. After flying a low approach to minimums, it flew north to fly the RNAV (GPS) runway 01 @ I68, which is the Warren country airport.

The aircraft again flew a low approach to minimums and departed to the south, with a plan to fly the RNAV (GPS) 29 back into Hogan Field. To do that, Cincinnati Approach issued a series of vectors, and ultimately told the aircraft to fly a heading of 260 and join the runway 29 approach course.

Later the controller said ?You appear to be kind of all over the place.? Subsequently, the controller declared an emergency for the pilot. The pilot was unable to fly the next instrument approach, so the controller ended up talking him down through the clouds.

Max talks about when to choose an IAF versus Vectors when Getting Vectors to Final. Like most things in aviation, the answer is ?It Depends.? In older GPSs, choosing Vectors drops some waypoints, which is fine over flat terrain, but which can be dangerous when vectored over mountainous terrain. This is why CFIs often encouraged pilots to always load an approach with an IAF and not vectors, even when being vectored for an approach. Newer GPSs have solved this problem, and in most cases, it?s fine to load an approach with vectors.

302 San Diego Cessna P210 Crash ? Poorly flown ILS and low fuel

Max talks about the crash this week of a Cessna P210 in San Diego, and why it crashed. The flight originated in Concord, CA and flew for just over 4 hours to Montgomery Field in San Diego. The aircraft first tried to land at French Valley airport, was high on the approach and went missed. It diverted to Montgomery field to fly the ILS 28R, however the pilot flew about a third of a mile south of the airport, missing the runway. Five minutes after the approach, while climbing for another approach, he ran out of fuel. ATC audio provided by LiveATC.net

301 San Diego Cessna 340 Crash ? the Perils of not Understanding Circling and Visual Approaches

Max talks about why the pilot of a Cessna 340 in San Diego turned right instead of joining the localizer and later crashed. It all came down to the pilot?s lack of understanding of how to fly a circling approach, and his inability to communicate that he?d like to switch to a visual approach. 300 San Diego Cessna 340 Crash Final NTSB Report ? Currency and Proficiency

Max talks about the final NTSB report results for a Cessna 340 that crashed in IMC while on approach into San Diego. The pilot?s logbook revealed that Instrument Proficiency Checks and Flight Review conducted during the prior year were cursory and did not meet the requirements for issuing those endorsements. Max talks about the problems with pencil whipping flight training and falsifying endorsements. 294 Diamond DA50RG Demo Flight - 300 hp FADEC Jet Fuel Engine

Max Trescott test flies the new single engine DA50RG aircraft that was recently certified in the U.S. by Diamond. Diamond has been working on different configurations of the DA50 for over ten years, and the 5-seat DA50RG with retractable landing gear is the first version to be certified and delivered in the U.S. It uses a 300 hp, FADEC controlled engine that runs on Jet A fuel. Base price for theDA50RG is $1,150,000 and options include air conditioning, FIKI, and oxygen.

Mike talked about that job, about his activity as a Private pilot and his thoughts on NexGen and the future of General Aviation. Mike talked about factors that led to the decline of GA after the 1970s, and his optimism for further growth due to demand for more pilots, the Part 23 rewrite, and new technologies like electric aircraft. Max talks with Catherine Cavagnaro about the new FAA Advisory Circular AC 90-66C on non-towered operations and straight-in approaches. The new Advisory Circular is significantly longer than the previous version, and it places much more emphasis on straight-in approaches. It says ?the FAA does not recommend that the pilot execute a straight-in approach for landing, when there are other aircraft in the traffic pattern.? Pilots choosing to execute a straight-in approach do not have a particular priority over other aircraft in the traffic pattern.

Pilots ?should not use paint schemes or color descriptions to replace the use of the aircraft call sign.? Also, ?Pilots are reminded that the use of the phrase ?ANY TRAFFIC IN THE AREA, PLEASE ADVISE? is not a recognized self-announce position and/or intention phrase and should not be used under any condition.?

287 Truckee Jet Crash: Unstabilized Circling Approach and Poor CRM + GA News

Max talks about the July 2021 crash of a Challenger Jet that was doing a circling approach at the Truckee, CA airport. The NTSB just released its final report on that accident, And there?s a lot that GA pilots can learn from this accident.

The NTSB faulted the first officer?s improper decision to attempt to salvage an unstabilized approach by executing a steep left turn to realign the airplane with the runway centerline, and the captain?s failure to intervene after recognizing the FO?s erroneous action. Both pilots both ignored stall protection system warnings, which resulted in a left-wing stall and an impact with terrain. The NTSB also cited the flight crew?s self-induced pressure to perform and poor crew resource management, which degraded their decision-making.

The flight was the first pairing of this crew with the operator. The captain, who had signed an employment contract with the operator, but was not yet an employee at the time of the accident. The first officer was not the accident operator?s employee at the time of the accident and had been hired as a contract pilot for the accident flight.

The pilots calculated that runway 20 was too short for the landing distance required by the airplane at its expected landing weight. Instead of making a request to ATC for the straight-in approach to runway 11 (the longer runway), the captain told the FO they could take the runway 20 approach and circle to land on runway 11. There is a straight-in RNAV (GPS) approach to runway 11, but the pilots didn?t request it.

286 Flying Relief to Maui's Wildfire Victims ? interview Laurence Balter

Max talks with Laurence Balter about how the general aviation community in Hawaii is flying relief supplies to help victims of Maui?s wildfires. The town of Lahaina is served by just a single road, which has closed frequently since the wildfires began a week ago. Electricity and cell phone outages have also hampered relief efforts.

Soon after the fires, local Maui pilots began organizing to fly supplies into Kapalua Airport, near Lahaina, but which is closed to most general aviation flights. The pilots received permission to fly into Kapalua, and began flying relief supplies to the airport.

Balter and Maui Brewing founder Garrett Marrero organized the effort via text messages and social media, and flights began on August 11. The pilot group worked with the airport fire departments at Maui's main Kahului Airport. Firefighters and volunteers weighed and loaded supplies that were delivered to the fire station. The process worked in reverse at theKapalua airport, where local fire fighters unloaded planes and helicopters and arranged for the distribution of supplies to local victims in Lahaina.

Laurence is flying his relief flights using the Cirrus SR22 that he usually uses for dual-led flying experiences, for pilots vacationing on Maui who are interested in flying a Cirrus and getting exposed to flying in Hawaii. He's also set up a Go Fund me page for people interested in donating to pay for avgas that volunteer pilots have had to pay for to fly the wildfire relief missions.

285 How to Choose a Flight Instructor and When to Fire a CFI + GA News

Choosing a flight instructor is one of the most important decisions you?ll make as you work to become a pilot, and later as you work on additional certificates and ratings. And that?s because having the right CFI can be one of the biggest factors in your success. Not that you can?t succeed with an awful flight instructor, but it?s so much easier if you?re with the right flight instructor.

Most likely, you?ll get to choose a new flight instructor multiple times, either because your first flight instructor has moved on, or you?ve moved to a new location, or you?re working on advanced training in some aircraft in which your previous flight instructor doesn?t specialize. And the flip side of all of this is firing a flight instructor, which we?ll talk about later.

Since choosing the right flight instructor is so important, you?ll want to give this a lot of thought and not just sign up with the first flight instructor you come across, or your friend, or your employee who?s been itching to get you to fly with them.

Unfortunately, in some cases, you won?t get to choose your flight instructor. If you learn to fly in the military or in a Part 141 school, you probably won?t have a choice of flight instructors. Though you will probably end up flying with multiple instructors. And that?s a good thing. As every CFI tends to put more emphasis on some things and less emphasis on others. And by flying with multiple instructors, you?ll pick up on nuances that your primary instructor never mentioned, or didn?t put much emphasis on.

Ultimately, when you?re working on a pilot certificate, or a new rating or certificate, or transitioning into a new aircraft, you?re working to assimilate a body of knowledge to help you become the best possible pilot. Your goal is not just to pass a checkride. Your goal is mastery of the aircraft. And to do that you?ll pull from many resources. You?ll try to be a sponge and learn as much as you can from books, online courses, and videos. And you?ll try to learn something from each of the flight instructors you fly with.

Choosing a flight instructor is not a precise science; there?s no textbook-like process or formula for choosing that person. Ultimately, it?s a subjective process, and that?s appropriate, because there?s no one flight instructor who?s perfect for everyone. A flight instructor who was ideal for a pilot friend of yours? may not be the right one for you for a variety of reasons.

284 The final NTSB report is out for Transair flight 810, which crashed at night in the ocean shortly after takeoff from Hawaii. The pilots thought that they had a rare double engine failure. In fact, they had just a single engine failure, but they left both throttles at idle, dooming the plane. The probable cause said ?contributing to the accident were the flight crew?s ineffective crew resource management, high workload, and stress.? Max talks about the huge distraction that consumed both pilots for a half hour, just moments before they took off.

The pilots spent 36 hours waiting to be rescued. At the same time, the Guard was trying to extract 12 mountain climbers and their rescuers who were stuck on a nearby mountain due to high winds.
